- COVID-19/ATD Screening Form: will be reviewed by staff at both pre screening telephone call and at time of your appointment.

Payment Policies
Payment is appreciated at the time of your visit. We accept Cash/Check/ Credit cards: Visa/MasterCard/Discover/American Express.
Kindly advise us of your insurance benefits at the time of scheduling your appointment. We will confirm eligibility and advise you as needed. We are happy to file your insurance claims as long as you are free to choose your own dentist and coordinate your treatment to fit your needs as best as possible.
Financial arrangements are available for more complex procedures and cases. Dr. Chin will discuss your treatment options during your diagnostic consultation. Please speak with our financial coordinator Pam, as our goal is to have every patient select the option best suited for their situation and ultimately to have their best smile for a lifetime.
What To Expect
Being well-prepared for your appointment will ensure that Dr. Chin has all of the needed information to provide the best possible care for you.
